Women’s Soccer Club — Minor Player Parental/Guardian Consent & Release of Liability
This consent and release must be completed by the parent or legal guardian of any player under 18 years of age before the minor may participate in Women’s Soccer Club (WSC) / Women’s Charities, Inc. (WCI) activities.
I, the undersigned parent or legal guardian, certify that I am the parent or legal guardian of the minor named below and have the legal authority to consent to her participation and to sign this release on her behalf.
In consideration of my minor child being allowed to participate in any way in the Women’s Soccer Club (WSC) and Women’s Charities, Inc. (WCI) athletics/sports program, related events and activities, I acknowledge, appreciate, and agree that:
- The risk of injury from the activities involved in the program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS on behalf of my minor child, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my child’s participation; and
- I willingly agree, on behalf of my minor child, to comply with the stated and customary terms and conditions for participation. If my child or I observe any unusual significant hazard during her presence or participation, we will remove her from participation and bring such to the attention of the nearest official immediately; and
- I, for myself, my minor child, and on behalf of our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS the Women’s Soccer Club and the Women’s Charities, Inc. and their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“Releasees”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT ON BEHALF OF MYSELF AND MY MINOR CHILD, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
Sportsmanship Agreement (on behalf of the minor player): I agree that my minor child will observe good sportsmanship at all times and adhere to the basic safety and sportsmanship rules outlined in the online WSC Handbook. I understand this is a “teaching” club open to all levels of play, and I agree my child will behave in a manner keeping with the Mission Statement and organizational goals of the Women’s Soccer Club, Inc. I understand that if she does not adhere to the rules set forth in the online WSC Handbook, she may be released from the WSC pending review by the Grievance and Disciplinary Committee.
